• Name of your Area; if not known, contact your District Commissioner or the provincial office at 1-800-565-8111.

• 2 pieces of legible government-issued identification with full name, one of which must include a photo.

• A valid credit card; the fee is $25 plus HST.

The following process must be completed:

• Find your Area below and click on it.

• You will be prompted to create a MyBackCheck account and pay the fee by credit card.

• When the invitation has been received, you will be required to enter your identity information.

• Print the MyBackCheck.com Consent forms upon completion and take them to your District Commissioner along with two pieces of identification.

• The District Commissioner will verify your identity by taking photocopies of the government-issued identification and will email the completed forms and copies of the ID to support@mybackcheck.com or by fax to 1-866-553-8012.
